You can go to your GP and show him. He will examine you. The doctor may then give you a referral. There are several ways to get an appointment, at least in Bavaria.

One way is a hotline. You call them, give them a code (this code is IMPORTANT), dictate what is written on the referral and they type it all into the PC. They then look for a doctor/practice for you where an appointment is possible. This is usually close to you, i.e. max. 20km away. I don't know if this is also possible in your federal state.
